The burden of chronic disease in the U.S. is placing great stress on our healthcare systems. This webinar, presented by nursing leaders from academia and practice, will examine workforce initiatives that are needed to address this challenge.
Learners will:
- Understand national initiatives to improve chronic disease management within primary care.
- Identify educational and transitional programs to promote the integration of chronic disease management into the competencies of new nursing graduates.
- Explore innovative models of care delivery that utilize technology and telehealth in the management of chronic disease.
